Why psychiatric placements are the scarcest of all
More nurses enrol in psychiatric tracks each year than there are certified psychiatric prescribers prepared to supervise them, and the gap has widened for a decade. The practical effect on you is a queue tilted in your favour. Offers reach you more frequently than they reach a colleague in primary care, and you get to pick the term, the weekday and the format.
It also means faculty are flexible with you in ways they are not elsewhere. Half blocks of 60 student hours get filled in psychiatry because a coordinator would rather divide one student across two prescribers than send them into a term with no placement at all.
None of that changes what you are paid. The rate band is the same for psychiatry as for anything else, and you set your own figure inside it rather than accepting one set for you.
The evaluation your student runs while you sit back
A psychiatric rotation turns on the initial assessment, so that is what you build the term around. After a student has watched a handful of yours, give them a new patient and move to the observer's chair. They open, they take the history, they ask the risk questions, they try a formulation out loud. You stay quiet unless safety demands otherwise, and you keep two teaching points on paper for afterwards.
Make them write the mental status exam in their own language before they read yours. Appearance and behaviour, speech, mood set apart from affect, thought process and content, insight, judgement, and cognition when the presentation warrants it. Students arrive able to list those headings and unable to apply them to a person sitting in front of them.
Setting your version next to theirs inside the same record does more than any explanation given in a corridor. Do that three times and most of the habit is fixed for good.
Prescribing reviews translate well to a screen
A medication follow-up is largely a conversation, and a camera does not degrade a conversation. Your student can carry the whole visit: what has changed since last time, sleep, appetite, adherence, side effects, the rating scale repeated, then a proposal for what to do next. You come in at the end to confirm and to sign.
This is the reason programs permit a larger share of virtual hours in psychiatry than in almost any other population. The share is still theirs to decide, not yours, and it is written into the affiliation agreement your practice signs with the school. Ask for the number in writing before you commit a term to it.
Practical points make the difference on video. Use a platform that supports a private breakout so you and the student can speak without the patient hearing. Tell the patient at the start of the visit that a student is present and record their agreement in the note. Have a plan for a dropped connection mid-visit, and agree beforehand who calls the patient back.
Risk, and the part you never hand over
Let the student ask the risk questions, because asking them badly is the commonest failure in a new psychiatric clinician and it only improves with repetition in front of someone who will correct it. But the decision that follows the answer stays with you, every time, in the room and on camera alike.
Walk through your own safety plan with them once, slowly, before they ever need it: who you call, what you document, how you arrange a same-day review, what changes when the patient is at home on a screen rather than in your building. Then have them draft one with you present, and edit it together.
Set the rule out loud on day one. If the student thinks a patient is unsafe, they interrupt you immediately, whatever you are doing. Students who are unsure of that rule sit on their worry until the end of the visit, which is exactly the wrong time to raise it.
How a psychiatric block is built and paid
| Commitment | Student hours | Payment |
|---|---|---|
| Half block | 60 | $720 to $1,200 |
| Full block | 120 | $1,440 to $2,400 |
| Your hourly figure | Set by you | $12 to $20 |
Two payments per block, both by direct deposit. The first releases once you have filed the midpoint evaluation, the second once the final one is done. There is no invoice at either point and nothing is taken from what you are owed.
Because psychiatric visits are often shorter than primary care visits, a psychiatric block can be finished in fewer calendar weeks than a family one. Two half days a week is a common shape. Look at how a term maps onto your own leave before you accept, since a fortnight away in the middle of a block is harder to recover from than it sounds.
What you need before you can say yes
An active license without restrictions, current national certification as a psychiatric mental health nurse practitioner, and two years in practice after the date you certified. Certification and population have to line up: a psychiatric credential teaches psychiatric students and nobody else.
Your prescribing authority is worth checking against your state's rules as well. What a student can be shown of independent decision making differs between a state that grants full practice authority and one that requires a collaborative arrangement, and faculty sometimes ask about it during site review.
After that it is scheduling and permission. Your employer signs the affiliation agreement, not you, and the coordinator collects the paperwork. Wider context on the lanes open to nurse practitioners sits on the NP jobs guide.
Questions
Can a psychiatric rotation be done entirely on video?
Sometimes, but the decision belongs to the school and it is written into the agreement with your practice, not negotiated with you. Some psychiatric courses permit a full virtual term, others require a set number of in-person hours. Ask the coordinator for the exact figure before you agree to a term, because rebuilding a schedule in week four is painful.
Do I need to be licensed where the student is sitting?
You need a license valid in whichever state the patient is sitting in, because that is the rule governing the care itself. The student's own permissions to participate come through their program and its agreement with your practice. If your patients are in more than one state, tell the coordinator early, since it can affect which visits your student is allowed to join.
How do I ask a patient's permission for a student on camera?
Say it plainly at the start of the visit, before any clinical talk. Give the student's first name and role, explain what they will be doing, and make clear that declining changes nothing about the appointment. Then write in the note that consent was given and by whom. Patients refuse far less often than clinicians expect.
Does this count toward my PMHNP recertification?
It can. ANCC accepts precepting done inside your own role toward professional development, provided a letter confirms the dates and the hour totals. AANPCB permits a capped quantity of precepting to stand for part of your non-pharmacology continuing education. Confirm the current requirement on your own board's page, and keep the letter with your renewal records.
